Appointments, Payments and InsuranceAppointment Scheduling
We take pride in providing an excellent value in physical therapy services. In order to continue to provide a high standard of care, we seek appropriate and prompt payment for services provided. We are very comfortable discussing our reasonable fees and hope you feel equally comfortable clarifying the cost of services at any time during your visit. Payment is expected at the time of service unless prior arrangements have been made. Discounts are available for payment made on the day of service. The person receiving physical therapy services is ultimately responsible for payment, and we are unable to bill an attorney for physical therapy services. Copayments, coinsurance, deductibles Our policy is to maintain a credit card on file with our credit card merchant service. Once entered, the identifying information is no longer accessible by any member of our staff. Copayments will be billed to the card on file for each day of service. If you have a deductible or coinsurance amount due, you will receive a mailed statement indicating patient responsibility after your insurance processes the claim. The balance will be billed to the card on file on the statement due date. You may contact us at any time to change your card on file or remit payment by another method in advance of the due date. Insurance Filing We recommend verifying your insurance coverage for physical therapy services before your visit. As a convenience, we will file insurance claims on your behalf. When possible, we request 24 hours notice for cancellation of an appointment. Late cancellations and failed appointments will be subject to a $25 charge. Insurance cannot be billed for cancellation fees. Late Arrivals We recognize your time is valuable and make every effort to see all clients at their scheduled time. Please understand that your therapy time may be shortened accordingly if you are late. Physical Therapy Interns Lakeshore Sports Physical Therapy does accept physical therapists in training who may, under the supervision of your physical therapist, participate in your care. Please discuss any concerns with your therapist regarding interns involved in your care. Facility Use Clients may use equipment only as directed by their therapist. Guests who may be attending your visit are prohibited from utilizing any equipment within the facility. Privacy Policies
